Informed Consent to Treat and Liability Waiver and Release

RELEASE OF ALL LIABILITY
In exchange of the fee paid by you, Christina Nelson and the practitioners of Calm Peaceful Life are providing a service assisting you in mind , body, spirit integration through modalities that may include but not limited to: breathwork, sound healing, guided meditations, chakra balancing, private consultations, yoga, wellness coaching and energy work. They do not give medical advice nor treat medical conditions.

You the participant , hereby agree to assume all the risks associated with any outcome resulting from your participation in Calm Peaceful Life Courses, or classes, or private consultation with their practitioners. Side effects may include, but not limited to: emotional release, lightheadedness, mild fatigue, physical tingling, and change in body temperature.

All information and mental or physical guidance and direction are provided during courses, classes or private consultations to enhance the well being of you, the participant. It is my responsibility to consult a medical doctor if you have any underlying conditions prior to the start of new modalities. It is my responsibility, as a participant to notify the practitioner ahead of treatment of any pre-existing medical conditions, injuries, electrical medical devices or accommodations needed. I will consult a medical doctor for any medical advice.

By signing up for Calm Peaceful Life Classes and agreeing to terms conditions and Liability you agree that neither Christina Nelson or Calm Peaceful Life practitioners or Calm Peaceful Life shall be responsible for any negative consequence of any kind which is claimed to result from:
1)Your participation in any of these courses, activities, or consultations,
or
2)from the use of any information learned or obtained during these courses, activities or consultations including but not limited to , breathwork, guided meditations, sound healing, chakra healing, energy work, yoga, wellness coaching, or private consultation provided during Calm Peaceful life courses, classes or private consultations.

you further agree not to assert any legal claims of any kind in any form against Christina Nelson or Calm Peaceful Life on any asserted negative consequences resulting from your participation in these courses, classes or private consultations.

you acknowledge and agree that Christina Nelson and Calm Peaceful Life, disclaim all liability for any injuries or negative consequence claimed by you to be the result of your participation in these programs, classes, or private consultations. Christina Nelson is not a medical doctor and does not give medical advice. Please consult a doctor if you need medical treatment.

By signing this waiver and Release for a course, class, event or workshop you grant permission to the rights of your image, likeness, and sound of your voice as recorded audio or video for promotional use by Calm Peaceful Life without payment or any other consideration.

Calm Peaceful Life's Vision is to provide a Holistic Health Resource that is practical, accessible and effective. We provide an avenue to help cultivate a self care practice and tap back in to natural healing. We believe that there is a strong connection to the mind, body and spirit to and individual and their overall well being. To that end we employ techniques such as breathwork, meditations, and sound therapy to promote relaxation and balance. We also provide coaching on stress reduction, nutrition, somatic movement and other holistic health therapies. Our hope is to provide a journey of expansion that will contribute to life-long holistic health, healing and happiness.
